Option 1 - Print out this electronic order form. BandOrderForm.pdf. Complete the form and send a personal or certified check payable to Band of Angels or a money order to the address on the form. Personal check orders will be held 7 days until the check clears. If you do not have Adobe Reader on your computer, you will not be able to read or view the order form. Most computers come with this already installed. If yours does not, you can download it for free by clicking on this link. 
Option 2 - Send an e-mail to bandofangels@wi.rr.com with your order request. We will generate a PayPal invoice as a reply. This will instruct you how to create a PayPal account (if you do not already have one) and electronically send payment directly to the Band for Angels Fund. Once payment is received, your shipment will be processed. If you use PayPal we will be charged a small usage fee, please try to use Option 1. This will increase the profit and generate more donation dollars.
Option 3 - INTERNATIONAL ORDERS ONLY a $15.00 shipping fee will be automatically added. Use this PayPal shopping cart and pay by credit card. International orders only! You can add both youth and adult size bands by returning to this page and changing the pull down size menu and then pressing the add to cart button below.

For all orders shipping outside the US or Canada, please e-mail your order request to bandofangels@wi.rr.com. We will e-mail you back with the information about the actual shipping charges.

To entice you to purchase extra bands, US orders of 50 or more bands will not be charged a shipping fee.
ALL ORDERS ARE SHIPPED BY US FIRST CLASS OR PRIORITY MAIL DEPENDING ON THE SIZE OF THE ORDER. SHIPMENTS ARE MADE ONCE PER MONTH, ON OR AROUND THE 20th OF THE MONTH. THANK YOU.
